The
major purpose of the Save An Eye Game is to provide free eye care
to low income children. The accompanying form can be used to apply
for help in obtaining eye exams and glasses for children.
Please list all sources of income for the immediate family of the child. Also, please list any unusually high expenses such as unpaid medical bills, etc. If you need additional space, you may use the back of the application form. Also, please indicate if there are any forms of eye care insurance such as medical assistance (Access Card), Med-Plus, Gateway, ION, or Chip. The suggested donations of $5.00 towards the eye exam and $10.00 towards the glasses for children are only suggestions. If you cannot provide a donation, please do not let that stop you from sending in the application. The important thing is to obtain the needed eye care for your child.
